Problems associated with surgical resection of hepatocellular carcinoma


Abstract
An increasing number of hepatocellular carcinomas have been detected with recent advances in diagnostic modalities. Cirrhosis of the liver is present in most patient with hepatocellular carcinoma, but significant hepatic resection is
contraindicated
because of less regenerative capacity and substantial risk of hepatic failure. In addition to this, HCC show early development of portal vein invasion and intrahepatic metastasis, resulting from peculiar hemodynamics that portal vein bear the
draining
system of the HCC instead of hepatic vein.
Based on these, surgical rescetion should be done to remove entire portal bed of tumor while sparing as much functioning hepatic parenchyme at the same time. To accomplish this purpose, extent of hepatectomy should be evaluated preoperatively by
ICG
test or other scoring method and intraoperative ultrasonography is essential.
